HISTORY
The Medical Social Service Unit was established in 2012. 1 Social Service Specialist provides services in the unit.
LOCATION AND CAPACITY
The unit located on the B-D block corridor;
- All social workers working in the hospital work affiliated to the unit.
- In clinics-services such as emergency, psychiatry, oncology, physical therapy, etc. where patients in need of social service intervention are frequently seen, and in units such as dialysis, child monitoring center, etc., when social workers are needed full-time or part-time, the social service unit supervisor makes planning according to the number of social workers in the hospital.
- Social workers cannot be sent to other units or clinic-services without the approval of the social service unit supervisor. If there is a need for a social worker in these units or clinic-services, it is requested from the unit with the Medical Social Service Unit Consultation Form.
- In the clinic-service or different service units, the necessary conditions for social workers to carry out their professional work are provided within the relevant clinic-service or unit.
- Patients who apply to the hospital and need social services during the medical treatment process are referred to the unit by the patient's physician in accordance with the Medical Social Service Unit Consultation Form.
- A Patient Interview Form is prepared for the patient with whom the social worker has a professional interview. Every interview process with the patient is recorded on the form.
- Forms are kept in the unit. When necessary, the Patient Interview Form is added to the patient file. If the patient has been referred by the physician as a result of the interview, information is given to the physician with the Medical Social Service Unit Consultation Form.
- Within the scope of social service intervention, social investigation is carried out at the hospital, at the patient's residence, workplace, school or social environment when necessary.
- The social worker conducting the investigation prepares the Social Investigation Report. The social worker ensures that this report is sent to relevant institutions and organizations as an attachment to an upper letter when necessary. One copy of the report is kept in the unit in accordance with confidentiality and one copy is placed in the patient file. Service requests from institutions outside the hospital are made with an official letter.
- The social service intervention performed is recorded in the Medical Social Service Unit Record Book for each day. Multiple records are not made for the same person within the same day.
- The work done by the Social Service Unit is prepared with the Medical Social Service Unit Activity Form or whenever requested by the ministry and routinely, by the end of the first week of January and July of each year and sent to the Mental Health and Social Diseases Branch as an official letter and e-mail. All forms are added to the hospital automation system. Records and reports are sent on time and regularly.
