HISTORY

The Health Board Unit was established in 2011. Our department provides services with 1 Officer and 2 Administrative personnel.

LOCATION

Our unit provides services on the Ground Floor of D Block.

  • Disability Unit (3-signed Status Report Health Board Reports)

WORKFLOW

Applications are received from our patients for Disability Report, Disability Retirement or the report they want to obtain. The petition or upper letter is recorded in EDMS. Then, after entry, patients are directed to departments for Disability (Home Care Fee, Benefiting from Disability Rights, Court Submission, Getting Class H License) to 7 departments-[PTR, OPHTHALMOLOGY, ENT, NEUROLOGY, GENERAL SURGERY, PSYCHIATRY, INTERNAL MEDICINE],

For Status Report (Disability Retirement, Weapon Carrying and Possession License, Being a Private Security Officer [Armed or Unarmed], Starting Work, School Registration, Assignment, Citizenship, Educational Visa) to 6 Departments –[OPHTHALMOLOGY, ENT, ORTHOPEDICS, PSYCHIATRY, INTERNAL MEDICINE, NEUROLOGY] and departments related to the diseases of our applying patients are opened and patients are directed.

After the examination is completed, it is checked and a report is prepared for the patient to enter the board. Once a week on Wednesdays, our patients who apply for Disability, Disability Retirement, Weapon License, Starting Work, Assignment, Court Submission and Status Report enter the board.

In the final stage, the patient's report is prepared together with the scores and diagnoses given by our Professors, and if it is a personal application, it is delivered to the Patient. If it is an Official application that comes with an upper letter, a response is written to the institution from which the upper letter came, and after the approval stage, it is sent to the institution together with documents related to the patient's disease and reports if any.

Patients are sent to our Disability Unit by the institution with an upper letter. After the patient's EDMS registration is made to one or more departments specified in the upper letter, they are directed. After the patient's examination is completed, when the 3-signed Status Report Health Board Report is delivered to us from the department, a response is written and sent to the institution after the approval stage.
